Support Now is our starting point for individuals across the lifespan.
You do not need to know exactly what service you need. You just need to begin.
Step 1: Create Your Account
To begin the intake process, click Create an Account and provide general demographic information. This step sets up your secure intake account in our system.
Please note: this is not the full intake form, but it is required in order to move forward.
After creating your account:

Check your email for a message from CentralReach to finalize your online account registration

Complete this step within 24 hours

If you do not see the email, check your spam or junk folder
Within two business days, you will receive a separate email from our intake staff with instructions to begin the full intake process.
If you do not receive that email, please check your spam or junk folder again, or contact us directly.

Step 2: Complete the Full Intake Packet
Once you receive instructions from our intake team, you will complete the required intake forms.
If at any point you feel unsure about what is being asked, our team is available to assist you.
Step 3: We Call You Within Two Business Days
After your intake packet is submitted, a member of our team will contact you within two business days.
During this call, we will:
Learn more about your concerns
Clarify goals and priorities
Answer your questions
Determine the appropriate next step
Schedule your first appointment
Our goal is to move you forward without unnecessary delays.
Step 4: Gather Supporting Documents
After your appointment is scheduled, we may ask you to provide documentation to help us prepare, such as:
Insurance information
Previous evaluations or diagnostic reports
School documentation, including an IEP or 504 plan, if applicable
You may gather and submit these documents while waiting for your appointment. If you do not have them yet, we will guide you through what is needed.
