Navigation and Transitional Services
We know that for many families, the most difficult part of the journey is not just accessing therapy or evaluation. It is understanding what comes next. Transitions between services, shifts in developmental needs, school planning, and preparing for life beyond formal supports can feel overwhelming.
Our Navigation and Transitional Services are being developed to provide structured guidance and coordinated support during these important stages.
What This Will Include
As this service line grows, it is expected to include:
Guidance around transitions into and out of center-based or outpatient services
Support navigating school systems and community resources
Planning for developmental milestones and life-stage changes
Assistance understanding care options and levels of support
Coordinated consultative services for families needing short-term direction
This program is designed to build on MAC Midwest’s existing clinical expertise while offering families a clearer path forward during periods of change.

Navigation services will reflect the same principles that guide all MAC Midwest programs: individualized planning, coordinated care, and clinical expertise grounded in real-world needs.
The goal is not to replace existing services, but to strengthen continuity and ensure individuals and families feel supported as they move through different phases of care.
Timeline & Availability
Navigation and Transitional Services are currently in development. While full programming is not yet available, families interested in learning more are encouraged to connect through Support Now. Our team can discuss current options and determine whether consultative support may be appropriate.
