Step 1: Create an account.
Follow this link to create an account in our record system – New Client Account. Our staff is happy to answer any questions. Contact MAC intake at: (952) 767-4215.
After you create an account, you will be emailed with instructions for completing the intake forms.

Step 2: Complete your intake packet.
Someone from the MAC team will respond with an email and directions for filling out an intake packet within 1 business day. You’ll need insurance documentation, school records, and any other previous evaluations.

Step 4: Schedule your appointment
Our intake staff will contact you after clinical review of your intake packet to schedule your appointment.
Plan your finances
The cost of caring for someone with autism spectrum disorder (ASD) is different for each family. Many times, we are asked about insurance options or other funding available for families with a child or children living with ASD. Below, we’ve provided a few resources to help you along the way.
